Hello Murchison Community!

Physicals are required to participate in any UIL sport. AISD Physical Forms in both English and Spanish can be found at this AISD page.

Please have these forms filled out by your child’s pediatrician and then scan it for your records.

Pay special attention to the front page. IF ANY BOX IS CHECKED YES, THEN IT NEEDS TO BE EXPLAINED AT THE BOTTOM.

The student must sign the form as well.

ONLINE forms must also be completed. Those can be found here: austinisd.rankonesport.com

You can enter as a guest and fill out both contact info and medical history forms.
